Patients can download a new patient packet from the Home page and complete the forms, then fax to (479)226-3148 or bring to your appointment.

What services do you offer?

Family Medical Care provides comprehensive medical services including wellness exams, Medicare physicals, injections, small procedures, flu shots, pneumonia shots, allergy testing, balance testing, full lab services, all served with a great bedside manner.

Do you take my insurance carrier?

The good news is that we accept all insurance carriers, and will become network providers if we happen to not be in network at this time.  For those without insurance, the discounted price is $125 per visit.
